This role provides counseling, assessments, crisis intervention, and ongoing behavioral health support within a primary care setting.Responsibilities• Provide clinical assessments, diagnoses, and individualized treatment plans• Offer individual, family, couples, and group therapy• Support medical providers through same-day consults and brief interventions• Participate in care coordination and patient-centered health home activities• Respond to crises and provide appropriate interventions• Maintain timely, compliant documentation• Collaborate with multidisciplinary team membersQualifications• Master’s degree in a mental health field• Active Missouri license: LCSW & LPC• BLS certification• Strong communication, empathy, and problem-solving skills• Ability to use Microsoft Office and EMR systemsJoin a mission-driven team making a difference in underserved communities.
