NURSE PRACTITIONER-MOBILE HEALTH UNIT (PART-TIME)

Job Highlights:
The Advanced Practice Provider for Live HealthSmart Alabama (LHSA) Mobile Wellness provides clinical oversight, delivers preventive health consultations, and ensures the safety, quality, and effectiveness of all clinical operations within the Mobile Wellness program. This role is responsible for on-site clinical decision-making, high-quality patient education, urgent result triage, and coordination of clinical pathways that support LHSA’s mission to improve community health statewide.
- Clinical Consultation & Direct Participant Care – Conduct comprehensive preventive health consultations for Mobile Wellness participants (typically 25-30 per event).
- Retinal Scan Review & Urgent Triage – Perform asynchronous review of retinal imaging results from Mobile Wellness screenings. – Identify abnormalities that require follow-up, including urgent or time-sensitive findings.
- Navigation & Follow-Up Support – Collaborate with the Lead Navigator to support participant follow-up care plans. – Provide clinical decision-making for complex navigation cases. – Confirm appropriateness of referrals to primary care, ophthalmology, or specialty services.
General Responsibilities:
To provide medical care and treatment within established scope of nursing practice to pediatric patients as assigned by and under the direction of the collaborating physician (Section 610-X-9 of the Alabama Board of Nursing Administrative Code). The collaborating physician shall be available for direct communication for consultation and referral or arrange for the same medical coverage by a physician who is pre- approved by the State Board of Medical Examiners in the same manner as the collaborating physician of record. To coordinate plan of care, discharge and specialty referrals as appropriate to meet the age specific needs of the pediatric patient. To plan and coordinate health care education for pediatric patient, family and other health care providers as needed. To perform comprehensive physical examinations and assessments. To interpret, analyze and evaluate patient’s medical record and social data. To perform and/or assist the physician with procedures per collaborative agreement. To coordinate and participate in performance improvement activities. To travel and work up to 100% of the time in the mobile health unit service areas.
Key Duties & Responsibilities:
1. Clinical Consultation Direct Participant Care.
2. Conduct comprehensive preventive health consultations for Mobile Wellness participants (typically 25-30 per event).
3. Review and interpret screening results, including: Blood pressure, Hemoglobin A1C, cholesterol/lipid panels, BMI and anthropometrics, smoking status, and social determinants of health (SDOH).
4. Provide tailored health education using evidence-based guidelines and plain-language communication.
5. Engage participants in motivational interviewing to identify and set actionable health goals.
6. Document all clinical assessments, education, and care plans in REDCap or designated LHSA systems.
7. Navigation Follow-Up Support Collaborate with the Lead Navigator to support participant follow-up care plans.
8. Provide clinical decision-making for complex navigation cases.
9. Confirm appropriateness of referrals to primary care, ophthalmology, or specialty services.
10. Support SDOH mitigation plans when issues require clinical review or prioritization.
11. Perform other duties as assigned.
Salary Pay Range: $85,600 – $139,000
Qualifications
Minimum Requirements:
Master’s Degree in Nursing with an Family NP or Adult Primary Care NP certification and three (3) year of related experience required. Work experience may NOT substitute for degree requirement.
Licenses / Certifications / Registrations:
Currently certified as an Advanced Practice Nurse (APN) with Certified Registered Nurse Practitioner (CRNP) Specialty by the Alabama Board of Nursing required.
Preferences:
- Experience in preventive medicine, primary care, family medicine, public health, or chronic disease management.
- Strong communication skills and ability to work with diverse, underserved populations.
- Proficiency in interpreting basic laboratory and screening results.
- Experience with motivational interviewing or patient-centered counseling.
- Ability to work in fast-paced, mobile environments with interdisciplinary teams.
- Requires travel to Mobile Wellness events across Jefferson County and other Alabama communities.
- Must be able to work outdoors in varying weather conditions.
- Requires standing for extended periods and lifting/transporting clinic equipment (with support).
- Occasional evening or weekend events.
